The Palliative Care Team provides end of life care patients and their carers.
Referrals can be made by doctors, nurses and all professionals on the wards, together with Community Palliative Care Nurses, Palliative Care services out of area and the Hospice.
Families and carers can request our involvement at any time via any health professional or by letter and telephone.
The Hospital Palliative Care Nurse Specialist Team are available Monday – Friday, between 9am and 5pm.

The discharge in-reach nurse is provided by St Margaret's Hospice in partnership with Marie Curie Cancer Care. This is a new role responsible for identifying terminally ill patients who are in hospital and nearing the end of their lives and who wish to be cared for in the community.
The nurse work closely with care professionals in the hospital and community, and with the end of life care coordination centre to ensure that care packages are in place so that patients can be discharged from hospital without unnecessary delays and return to their preferred place of care.
Any health or social care professional in the hospital or community can refer a patient or a referral can be made by the patient themselves, their family or carer. Contact 07531 193642
